The annual CMS Medicare Part D Creditable Coverage Disclosure was completed on January 13, 2026. This required disclosure confirms that the prescription drug coverage offered under the plan is creditable (or non-creditable) compared to Medicare Part D and is submitted annually to the Centers for Medicare & Medicaid Services (CMS) as part of ongoing compliance requirements.
