Dental Care Required Due to an Injury 100% of Covered Expenses up to $200 per Trip Period

Please note: Coverage exclusions include, but are not limited to, routine maternity care, mental health, substance abuse, congenital conditions, self-inflicted injuries, injuries due to drugs and alcohol, infusion therapy, radiation therapy, hemodialysis, and outpatient physical therapy. Review your certificate for a complete list of exclusions.
