Please notify Blue Cross and MBA of your 2026 renewal decision - even if you are not making plan changes.
A copy of the plan election form and administrative fee disclosure has been attached for your review and signature.

Setup time: ~2 weeks. Opt in by checking "Yes" on the bank selection form.
