Please note: Coverage exclusions include, but are not limited to, routine maternity care, mental health, substance abuse, congenital conditions, self-inflicted injuries, injuries due to drugs and alcohol, infusion therapy, radiation therapy, hemodialysis, and outpatient physical therapy.
