More than one in three adults live with multiple chronic conditions. Without appropriate support, these conditions can worsen over time, leading to more serious health outcomes and higher overall costs of care.

This program is designed to help individuals living with chronic conditions improve their health outcomes through personalized, high-quality support for prediabetes, diabetes, hypertension, weight management, and mental health concerns.

What’s included

Eligible members receive easy-to-use health monitoring devices—such as connected glucometers and blood pressure cuffs—and work one-on-one with a certified personal coach to set and achieve health goals.

Members also have access to licensed therapists for mental health support. The program uses personal health data, social determinants, individual preferences, and clinical needs to drive timely, actionable outreach that supports consistent healthy behaviors and improved outcomes.

A whole-person approach

Teladoc Health’s whole-person approach to chronic condition management provides meaningful support for individuals facing multiple health challenges. By addressing physical and mental health together, the program helps improve clinical outcomes and lower the overall total cost of care.
